Update on intraoperative radiotherapy: new challenges and issues

چکیده
Intraoperative radiotherapy (IORT) for breast cancer has challenged the standard external beam radiotherapy (EBRT) and has been shown to be non-inferior for treating early breast cancer in the past decade. Several technologies have been tested for IORT and various randomised controlled trials are still ongoing. Different methods of application of IORT have also been evaluated, from early breast cancer to tumour bed boost radiotherapy amongst high risk women. TARGIT-A and ELIOT trials have reported a low incidence of local recurrence and good survival in both arms. Moreover, mortality has been found to be lower amongst women who underwent partial breast radiotherapy compared to those treated with EBRT in a recent meta-analysis. Despite this, IORT has not been introduced in the current clinical practice as yet, and many clinicians do not mention this treatment option to patients awaiting breast cancer surgery. The scientific community does not unanimously support the effectiveness of IORT and still raises concerns about introducing IORT as a standard treatment option for breast cancer. Current evidence demonstrates that IORT is ready for roll-out; it is time to let well-selected and informed patients be offered this treatment option in the current clinical practice.

Commissioning the First Mobile Dedicated Accelerator for Intraoperative Electron Radiotherapy in Iran
Introduction: Intraoperative radiotherapy is a radiotherapy technique in which a high single fraction of radiation dose is delivered to the patient after surgery and Concurrent with anesthesia time. The most frequent method for IORT implementation is Intraoperative electron radiotherapy (IOERT), in which, some dedicated and high dose per pulse electron accelerators are employe...

Introduction: The received dose to the surface of tumor bed is one of the main issues in beast intraopeative radiotherapy (IORT). Totally, the administered dose to the surface is usually lower than that of radiotherapy reference point (distal end of tumor bed). Employing a buildup bolus can effectively increase the surface dose and therefore, eliminate such created cold points ...
متن کاملAn Update of Couch Effect on the Attenuation of Megavoltage Radiotherapy Beam and the Variation of Absorbed Dose in the Build-up Region
Purpose: Fiber carbon is the most common material used in treating couch as it causes less beam attenuation than other materials. Beam attenuation replaces build-up region, reduces skin-sparing effect and causes target volume under dosage. In this study, we aimed to evaluate beam attenuation and variation of build-up region in 550 TxT radiotherapy couch.Materials and Methods: In this study, we ...
متن کاملPacemaker and radiotherapy in breast cancer: is targeted intraoperative radiotherapy the answer in this setting?
We present the case of an 83 year old woman with a cardiac pacemaker located close in distance to a subsequently diagnosed invasive ductal carcinoma of the left breast. Short range intraoperative radiotherapy was given following wide local excision and sentinel node biopsy. The challenges of using ionising radiation with pacemakers is also discussed.

Over one-quarter of a million cases of breast cancer are diagnosed in the United States each year, many of which are early stage. The radiotherapeutic options after breast-conserving surgery in early-stage breast cancer are evolving quickly, with a focus on minimizing treatment volume, toxicity, and treatment duration. One such emerging option is intraoperative radiotherapy (IORT), administered ...
